USD 308 School Health Services

The purpose of the school health program is to promote the health and learning of all students.

School nurses will work collaboratively with students, parents, health providers, teachers, and administrators to support school attendance and maximum learning, while  following, state and district regulation as well as the Nurse Practice Act.

Health & Nursing Forms for Parental Use:

Action Plans

  • Allergy Action Plan -  If your child has a history of allergies to food, environmental, and/or bee/wasp sting that require emergency treatment. The parent/guardian AND Physician is responsible for submitting the completed form as well as the prescription medication form to accompany any medications at school.
  • Asthma Action Plan - When a prescription medication to treat asthma is to be given/maintained at school to a student during the school day, the parent/guardian AND Physician is responsible for submitting the completed form as well as the prescription medication form to accompany any medications at school.
  • Diabetes Action Plan - The following forms and/or medications need to be completed by a parent/guardian and physician. Parent /guardian must make arrangements with the school nurse to meet one week prior to the first day of school or after diagnosis to develop an individualized health care plan to ensure the safety of your child while at school. Emergency medications such as glucagon, insulin, ketone strips, glucose tabs, and extra pump/insulin/glucometer supplies should also provided.
  • Seizure Action Plan - If you child has a history of seizures, the parent/guardian AND Physician is responsible for submitting the completed form as well as the prescription medication form to accompany any medications at school.

  • Community Resources - Hutchinson Resources Available to USD 308 Families
  • Dental Form- Prairie Star - USD 308 has chosen to participate in the Kansas School Sealant Program. Dental professionals will be offering services in your child's schools such as: sealants, fluoride varnish, and/or cleanings. There will be no cost to you or the school for these services. The following form needs to be completed to receive the free services.

Meal Modifications

  • Meal Modification Discontinuation - If your child has a physician order to discontinue a meal modification at school the parent/guardian AND Physician is responsible for submitting the completed form
  • Meal Allergy Letter - A letter from USD 308 Director of Nutrition Services
  • Meal Modification Form - If your child has a food allergy/intolerance that require meal modifications at school the parent/guardian AND Physician is responsible for submitting the completed additional forms.
