How?

(1) Click the link below. Print and complete.

(2) Bring that completed form and your insurance card to the flu shot clinic

(3) A nurse will administer your vaccination

 

Why?

  • The district nurses HIGHLY recommend it, as does your family doctor***

  • The flu is more serious than you may realize

  • Getting the flu shot protects you, your family, your friends and the community

  • Flu shots need to be re-administered annually

  • The process is simple, and it is cheap, possibly completely covered by your insurance company

***If you have any chronic medical conditions or concerns if you can get the vaccination, consult your primary doctor.

Remaining 2018-19 Flu Shot Clinics

  • Oct. 22: Wiley, 1:30 to 4 p.m.

  • Oct. 26: Support Services Center, 8:30 to 9:30 a.m.

  • Oct. 26: Administration Center, 10 to 11:30 a.m.

  • Oct. 26: HMS-8, 1:30 to 4 p.m.

  • Oct. 29: Graber, 1:30 to 4 p.m.

  • Oct. 30: HMS at Allen, 1:30 to 4 p.m.

  • Nov. 5: Faris, 1:30 to 4 p.m.

Hutchinson High School’s clinic was held earlier this week. If students or district staff miss a clinic, they can attend another, go to their physician or many pharmacies to get the vaccination.
